WSMC Downieville
Nutrition Education can be an important step in managing or preventing chronic diseases. You can work with our Dietitian one-on-one to develop a healthy nutrition path for you based on your unique needs. Speak with your medical provider about a referral to our Nutrition Services and take a step towards healthier living and we will be there with you along the way. Our dietitian also offers a weight management support group.
Hospice Care
Hospice care is provided as needed for our patients upon request. Western Sierra Medical Clinic, Downieville, provides informal Hospice Services in coordination with the patient’s Primary Care Physician. We also coordinate hospice referral to Hospice of the Foothills in Grass Valley and Journey’s Hospice in Camptonville.
House Calls
House Calls are available in the event individuals are unable to transport themselves, have no available transportation or are so ill as to make travel difficult, impossible or a significant medical risk. House Calls can be scheduled and coordinated by the Nursing Staff and the Medical Provider.
Home Health
Home Health Care is not available at this time. Medical Providers try to fill this gap as much as possible with House Calls. Private individuals are available for direct patient care and in-home support services on a fee for service basis. Sierra County Social Services also has a program that pays for in-home support services for those individuals who meet the income guidelines.
Food Bank
The Western Sierra Food Bank provides food to individuals upon request and is housed at the Immaculate Conception Church in Downieville. Access is coordinated by Clinic Staff upon request. In addition, there are three large food distributions provided to families and individuals on Thanksgiving, Christmas and Easter.
